REQUIREMENTS FOR A CHILD(<16 Yrs) US PASSPORT NEW OR RENEW

1. Completed Passport Application (DS-11  click on it to print).
   For Emergency service we need copy of application to be faxed before you sending the documents.

2. Two recent passport photos: 2”x 2”, frontal view, white background.

3. Proof of U.S. Citizenship, such as: Original Birth Certificate or Original Naturalization  
  Certificate and Old Passport (if child has earlier a Passport)

4. Parents Proof of Identity that has picture and was NOT issued recently, such as: State or
  Federal ID card, Drivers License, etc. (This will not go in Sealed Envelope)

5. Completed and signed
Authorization Letter  (click on it to print).

6. Proof of Applicant’s departure date from USA, such as: Copy of Applicant’s actual airline or
  cruise ticket or Travel itinerary from travel agent or Company letter explaining trip info.        

7. Passport Fee $120.00 (US Department of State) + $25.00 (Local PAAL Officer)
  (Check or Money orders only)
Procedure:

1. Go to a Passport Application Acceptance Location (PAAL)  with all of the above specified
   items. PAAL is usually a designated Post Office.
   Both Parents and Child must appear personally. If not call us for more information.

2. Request the PAAL Officer to do the local Processing and Verification of your Application.

3. Advise the PAAL Officer that WE will walk thru your Application Processing at the US State
  Department.

4. Request that all the appropriate items be placed in a SEALED ENVELOPE, and this
  envelope be given to you.

5. Write Applicant’s name, date of birth and departure date on the SEALED ENVELOPE.

6. Send the
SEALED ENVELOPE along with  Our Service Charges  by overnight delivery to:
