The US State Department has instituted new procedures for name changes on
US passports beginning Monday, September 26th, 2005. If passport issued
more than 1 year old you need to apply as Renewal Passport.
For less than one year old Passport follow the instructions here:
1. Complete & signed Passport Application (DS-5504 click on it to print).
2. Two recent passport photos: 2”x 2”, frontal view, white background
3. Applicant’s most recent US Passport less than one year old.
4. Completed and signed Authorization Letter (click on it to print).
5. Proof of Name change such as: Original Marriage certificate, Original Court Order Etc.,
6. Proof of Applicant’s departure date from USA, such as: Copy of Applicant’s actual airline
ticket or Travel itinerary from travel agent or Company letter explaining trip information.
7. Passport Fee $60.00 + Our Service Charges.
