3. Applicant’s most recent US Passport which is issued less than 15 years ago and the applicant
was 16 years or older. (not acceptable if this Passport damaged)
4. Completed and signed Authorization Letter (click on it to print).
5. Proof of Applicant’s departure date from USA, such as: Copy of Applicant’s actual airline
ticket or Travel itinerary from travel agent or Company letter explaining trip information.
6. Passport Fee $135.00 + Our Service Charges.
