3. Two recent passport photos: 2”x 2”, frontal view, white background.
4. Applicant’s valid US Passport (not acceptable if this Passport damaged)
5. Completed and signed Authorization Letter (click on it to print).
6. Proof of Applicant’s departure date from USA, such as: Copy of Applicant’s actual airline
ticket or Travel itinerary from travel agent or Company letter explaining trip information.
7. Passport Fee $135.00 + Our Service Charges.
